We just received word that the department has waived the June 30th deadline.  Effective immediately you must complete your medical exam during your birth month.  You do not have to complete it prior to June 30th unless you have a June birthday.

SUBJECT: FITNESS FOR LIFE PHYSICAL

 

 

Effective April 24, 2007, all employees in the Lake Lifeguard series (i.e., Lake Lifeguard, Senior Lake Lifeguard, Supervising Lake Lifeguard and Lake Aquatics Manager) are required to schedule a physical exam to retain the “Fitness For Life” bonus. The date for completion has been extended to JUNE 30, 2007.

 

The physicals must be conducted on their own initiative on (non county) time. There will be no prompting or personal notification from the Department. The exam may be taken free of charge at a county approved clinic or by a personal physician at the employee’s expense or medical plan. There are several forms to be completed in advance and special requirements for personal-physician exams. These items can be downloaded at www.local1014.org, click on Fit for Life under the main menu. Listed below are the required forms.

 

After July 1, 2007 the lifeguards will be required to get their physicals done during their birth months. They will have fifteen days before and after your birth month to complete the physical. If not completed in 2008 they will lose their 3% Fitness for Life bonus. List of Clinics

 

You will be notified at a later date of the CE (continuing education) requirements.
